Before I prepare your foundational kit and we begin our first exchange, please take a quiet moment to review and sign the Service Agreement and Informed Consent below, as well as fill out the Intentions form. If you're short on time, a link to the agreement will also be included in your order confirmation email—just note that both need to be completed prior to your foundational kit being sent.
